The Health and Social Care Regional Integration Fund is a five-year Welsh Government programme running from April 2022 to March 2027 to help with integrating health and social care services. The RIF is the main revenue fund for Gwent RPB.

What is the RIF?

Since its establishment, Gwent RPB has tested and developed key models of integrated care that now provide essential services as part of our health and social care system.

The RIF has allowed us to embed these models of care into core services whilst maintaining some scope for the continued development of new ones.

The fund is a key lever to drive change and transformation across the health and social care system in Gwent. Our aim is that by the end of the five-year programme we will have:

• Established and mainstreamed at least six new national models of integrated care
• Developed an effective and seamless service experience
• Accelerated health and social care development and demonstrating their value.

Who does the RIF support?

The RIF supports:

• Older people with complex needs
• People with dementia
• Unpaid Carers
• Children with complex needs
• People with emotional and mental health wellbeing needs
• People with learning disabilities and neurodevelopmental conditions.

What are the RIF models of care?

The fund outlines a set of Models of Care. Every RIF-funded project must align to at least one Model of Care. Each Model of Care is described below.

In Gwent, we invest the RIF to deliver Models of Care 1 to 5 and use our Capital funding to deliver accommodation based solutions.
